Hospital Discharge Service

How do you refer a patient to
Sefton Hospital Discharge Service?

This service operates from Monday to Friday during normal
office hours except for bank holidays. Referrals received between
9am and 3pm will receive a response on the same day. Referrals
received outside these times on a weekday, or over the weekend,
will receive a response on the next working day.
Referrals should be made using the referral form which is available
on request from hospitaldischargesefton@seftoncvs.org.uk

Can I Self-Refer to the Service?

Yes — if you have been discharged from hospital or a similar
health setting and need some basic support such as a one-off
shop, help to complete forms or welfare telephone calls, you can
contact us direct by phone or email and request a call back from
the Hospital Discharge Service
