Health and Wellbeing Trainers
The Health and Wellbeing Trainers provide personalised rehabilitative support to service users to enable them to return to full independence.This includes interventions for timely and safe discharge from hospital and the assessment of needs to avoid unnecessary dependence on more acute, long term services.
They work in the 4 locality areas of South Sefton - Bootle, Maghull, Crosby and Seaforth/Litherland.
The Health and Wellbeing Trainers support people to do things for themselves. People using the service are expected to set their own goals and work with the Trainers towards achieving them.
The Health and Wellbeing Trainers encourage behavioural change and motivational techniques to help achieve goals and develop confidence, they provide signposting to other appropriate services and help to develop social networks.
The Team can support with advice in a range of areas including finance, accommodation, social inclusion, health and relationships.
The Health and Wellbeing Programme is linked directly to the Integrated Care Teams (ICT) and patients can access the Health and Wellbeing Programme via this route or through a GP
or another health professional, VCF sector.
