Health & Social Care Forum - Wednesday 17 May

At this forum meeting we will hear an introduction to the Sefton Place Plan, it’s purpose, how it’s been developed and key themes/priorities. Attendees will have an opportunity to comment, sense check and ask questions. Within the plan we will focus on what we mean by Community Resourcefulness, share proposed definition and discuss language with a view to shaping the definition that will be agreed and adopted by the Sefton Health and Care Partnership.
 
Another element of the transformation of the health and social care partnership arrangements will be the reintroduction of Care Quality Commission (CQC) Assessment of adult care in borough.   After lunch attendees will be invited to participate in a workshop looking at  how the new assessment process could work and what Sefton will be doing in this first instance in regard to self-assessment.
